Why does back acne happen?

A combination of factors: more sebaceous (oil-producing) glands on the back than most of the body, chronic friction from clothing, sweat that lingers in the area, and sometimes hormonal triggers (especially testosterone-related). It's not caused by hygiene.

Are back extractions different from face extractions?

Slightly. Back skin is thicker, so steam time is longer (12-15 minutes vs. 8-10 for face). The extraction technique is the same. Cysts and nodules on the back, like on the face, should not be extracted — those need cortisone injection or topical treatment.
